1. Accomplished OIC-EW Licensure Examination Form.

  2. Birth certificate showing that the candidate is not less than 18 years of age (PSA authenticated)

  3. Approved seagoing service on ships of 750 kW propulsion power or more:
    1. Not less than 12 months as part of an approved education and training program, as Engineer Cadet, Engineer Apprentice, or otherwise as a trainee for certification as OIC-EW; OR
    2. Not less than 36 months in the engineering department; (ex. Fitter, Oiler, Motorman, Wiper)

  4. Approved Training Record Book (TRB) as proof of onboard training that meets the requirements of Section A-III/1 of the STCW Code, if in compliance with 3.1

  5. Valid Medical Certificate, which shall not be more than 2 years from the date of issuance, from DOH-accredited clinic/hospital with QSS in place, in accordance with Regulation I/9 of the STCW Convention; and

  6. Transcript of records with S.O. Number, from an accredited MHEI, as proof of successful completion of the approved education and training program that meets the standard of competence specified in Section A-III/1 of the STCW Code, or as otherwise approved by the Administration.

  7. For BSMarE students whose education and training started BEFORE June 2013: Proof of successful completion of OIC Updating Training Part A.

  8. Seaman’s Identification Record Book (SIRB)

  9. Notarized Crewlist from the vessel stamped by the Philippine Coast Guard (for Domestic Trade only)
